Sport physicals

Our qualified medical staff will provide the necessary documentation that your school requires. This service is provided to non-members of the practice as well. Immediate access to appointments available. A guardian must be present for the appointment.

A physical examination must be completed on or after April 15, 2023 for the 2023-24 school year.

 
 
 

  • A head-to-toe exam is completed including a thorough cardiac exam. Vision screening and vital signs are assessed. If findings are abnormal, recommendations for appropriate follow-up will be given.

  • • $45 for members
    • $50 for non-members
    • Additional charges may apply if additional testing needed, i.e., EKG, lab studies

  • Please bring a guardian and a completed MHSAA medical history form. If you have a prior history of a cardiac condition, bring a copy of your separate clearance from cardiology.
