The Direct Primary Care (DPC) model is a more affordable way to become a member of the practice. Patients pay a monthly or annual fee based on their age, plus modest fees for visits. For patients with a high deductible insurance plan, DPC offers significant cost savings. This is not health insurance and does not bill your health plan for any charges. It can be used by individuals as well as employers looking to offer employees cost-effective, transparent healthcare. DPC membership is available to women, men, and adolescents.

Patients can expect an exceptional standard of care with access to same-day or next-business-day appointments for urgent matters, longer appointment times, little-to-no-wait times, convenient technology and membership in a practice dedicated to a high level of customer care. Angela is a member of the Obesity Medicine Association and is certified as a Menopause Practitioner with the North American Menopause Society. She has 20 years of experience in primary care.     • Laboratory testing, imaging, specialist referrals and hospital services billed to your insurance as usual – or – option for direct bill in-office lab testing when available at greatly reduced rate.

  • Monthly Investment
    Age 25 and younger: $25 per month
    Ages 26-44: $45 per month
    Ages 45-64: $65 per month
    Age 65 and older: $85 per month

    DPC Visit costs
    60-minute visit: $85
    30-minute follow-up or acute visit: $45
    15-minute follow-up or nurse‘s visit: $25

    Patients can apply visit costs to their HSAs if available. Any testing or lab work may be submitted to insurance if applicable or patients will have the option to pay cash at a significantly reduced rate, often 90% less.
